Package is applicable for non-Muslim women only, age 21 and above.
配套只适用于 21 岁及以上的非穆斯林女性。

Subsequent year egg freezing maintenance fee is RM 1200/year.
次年起凍卵維護費為每年RM1,200.00。
- Excludes the cost of preliminary examinations such as semen analysis, hormone examinations if needed, ultrasound scans and first consultation fee.
套餐不包括前期檢查費用，如精液檢查、激素檢查、掃描和初次見診。

Any extra items or services required outside of this promotional package can be purchased as an add on according to Alpha IVF’s price list.
此套餐之外所需的任何額外服務和藥物都可以根據阿兒法的單價表添加。
- In the event that patient has abandon cycle due to medical reasons upon follow up on stage 2, remaining payment of RM 2000 (on stage 2) and RM 2000 (on stage 3) are voided automatically.
如果患者喺第2階段隨訪時因醫療原因放棄繼續療程，剩餘嘅RM 2,000.00 （第2階段）同RM 2,000.00 （第3階段）將無需付費。

In the event that patient fails to proceed to oocyte pick up on stage (stage 3) due to medical reasons, remaining RM 2000 (stage 3) are voided automatically.
如果患者因醫療原因未能在階段（第3階段）進行取卵，剩餘嘅RM 2,000.00 （第3階段）將無需付費。
- Package is valid for participating specialists – Dr Leong Wai Yew, Dr Haris Hamzah, Dr Tan Chong Seong and Dr Lam Wei Kian.
套餐僅適用於見診–梁伟耀医师、夏里士。 韩查医师、陈钟雄医师同蓝伟監医。
- Payment paid is strictly non-refundable and non-transferrable.
已支付的款項不可退還且不可轉讓。
- Alpha IVF reserves the right to change the terms & conditions should any circumstances and regulations change.
如果任何情況和法規發生變化，阿兒法保留更改條款和條件的權利。
- Package valid until 31 December 2022.
